Short answer: traditional hookah tobacco doesn't contain THC, so it can't produce a cannabis-style high. What it can produce is a nicotine buzz and carbon monoxide exposure — and the two feel similar enough that people often mistake one for the other. Does Shisha Get You High? First, Define "High"

People often use the words high, buzz and head rush as if they mean the same thing. They don't.

A standard bowl of tobacco-based shisha may create the first two experiences, but not the third. The distinction matters because symptoms that users casually describe as being "high" can actually reflect reduced oxygen delivery from carbon monoxide exposure.

The U.S. Food and Drug Administration states that carbon monoxide absorbed during hookah use can contribute to light-headedness and hypoxia. The U.S. Centers for Disease Control and Prevention also emphasizes that the water does not remove the high levels of toxic agents in the smoke.1, 2

Effects of Hookah Use: What Happens to the Body and Mind?

The effects depend on what's in the bowl, the nicotine concentration, whether charcoal is used, session length, ventilation, puffing pattern, and the user's health and tolerance. This makes one person's experience a poor predictor of another's.

Common short-term effects

  • Temporary light-headedness or a "head rush"
  • Relaxation or increased alertness
  • Faster heart rate and a temporary rise in blood pressure
  • Coughing or throat irritation
  • Headache
  • Nausea, especially in people with little nicotine tolerance
  • Dizziness or weakness

Nicotine can feel stimulating while also being described as relaxing. Part of the perceived relaxation may come from the social setting, slow breathing pattern and — in regular users — the relief of nicotine withdrawal. It should not be interpreted as evidence that hookah is calming or harmless.

In a double-blind, placebo-controlled study of 37 occasional waterpipe users, a 45-minute tobacco session increased average plasma nicotine concentration by 3.6 ng/mL and heart rate by 8.6 beats per minute. The tobacco-free condition produced almost no nicotine increase. However, carbon monoxide exposure rose substantially in both conditions, showing that removing tobacco did not remove the charcoal-related exposure.3

When a "buzz" may be a medical warning

Carbon monoxide is colorless and odorless. Headache, marked dizziness, vomiting, confusion, chest pain, shortness of breath, loss of coordination or fainting should not be treated as a normal hookah experience. Stop the exposure, move to fresh air and seek urgent medical help. Loss of consciousness, chest pain, breathing difficulty, seizures or severe confusion require emergency care.

What Substances Are in Hookah Smoke?

Hookah smoke isn't simply flavored water vapor. Its chemical mixture varies by tobacco, flavorings, heat source, temperature, hose, device and user behavior. Researchers and public-health agencies have identified several important classes of substances.

A laboratory review of waterpipe smoke identified nicotine and 27 known or suspected carcinogens. The exact dose reaching a user isn't fixed, but the variety of toxicants is one reason "smooth" smoke shouldn't be confused with safe smoke.4

What about tobacco-free or herbal shisha?

A genuinely tobacco-free product shouldn't supply tobacco-derived nicotine, but it can still produce carbon monoxide and other toxicants when heated with charcoal. In a crossover study, tobacco-free waterpipe products did not show reduced yields of carbon monoxide, nitric oxide, PAHs or volatile aldehydes compared with the tobacco product tested. Nicotine was the key exception because the tobacco-free preparation didn't contain tobacco.5

What changes with electric heating?

Removing burning charcoal removes one source of carbon monoxide and combustion-related pollutants, but it doesn't make tobacco safe. Exposure still depends on the material being heated, heater temperature, session length and inhalation behavior. Tobacco-based molasses can still deliver nicotine, and heating glycerol, sugars and flavorings can create other compounds. Charcoal-free should be treated as a description of the heat source — not as a "risk-free" claim.

Can Hookah Be Addictive? Addiction and Dependency

Yes. Tobacco-based shisha delivers nicotine, and nicotine can create dependence. The social, occasional appearance of hookah can hide the process because use may begin only at cafés, parties or weekends. Dependence is about the relationship with nicotine, not whether the product looks like a cigarette or is used every day.

Signs of possible nicotine dependence

  • Craving hookah or planning social activities around it
  • Using more often, for longer sessions or alone
  • Needing more nicotine to get the same effect
  • Feeling irritable, restless, anxious or unable to concentrate when not using
  • Trying to cut down but repeatedly returning to use
  • Continuing despite health, work, financial or relationship problems

A review in Tobacco Control concluded that waterpipe tobacco use supports nicotine dependence because it delivers nicotine, can suppress withdrawal and craving, and is associated with failed quit attempts among some users.6 A later study using the natural abstinence period during Ramadan found that waterpipe users reported abstinence-related urges and withdrawal symptoms comparable in important respects with those reported by cigarette users.7

Hookah Compared With Cigarettes, Vapes and Cannabis

No single "one hookah equals X cigarettes" statement can measure total health risk. Sessions, devices and puffing patterns vary. Comparisons should separate smoke volume, nicotine, carbon monoxide and psychoactive effects instead of combining them into one dramatic number.

The CDC estimates that a typical one-hour hookah session can involve about 90 liters of inhaled smoke — roughly 100 to 200 times the smoke volume of one cigarette — and may expose a user to nearly nine times the carbon monoxide and 1.7 times the nicotine of a single cigarette.2 This does not mean one hookah session is scientifically equivalent to smoking 100 or 200 cigarettes. It's a smoke-volume comparison, not a complete risk conversion.

Cannabis is different because THC directly affects brain functions involved in memory, attention, decision-making, coordination and time perception. If cannabis is smoked, its smoke can also expose the lungs and cardiovascular system to irritants and chemicals found in other smoke.8

What Is "Weesha"? Mixing Cannabis Into a Hookah

"Weesha" is a slang term — a blend of "weed" and "shisha" — for combining cannabis with hookah tobacco in the same bowl. People ask about it because they want to know whether adding cannabis changes the answer to "does hookah get you high?" It does, but not in a simple way, and it adds risks on top of the ones already covered above.

Any high still comes from THC, not the hookah

As the comparison table above shows, a hookah itself — the water, the coal or heating element, the hose — has no psychoactive effect. If a THC-containing product is added to the bowl, any cannabis-style high comes from that THC. The hookah is simply the delivery method, and it's not an efficient one for this purpose.

Why a hookah delivers cannabis differently than other methods

THC vaporizes at a specific temperature, roughly 157°C (315°F). Devices built for cannabis — vapes, joints, pipes — are designed to reach and hold that temperature against the plant material directly. A hookah bowl isn't: charcoal or an electric heating element sits above the bowl and heats its contents indirectly and more slowly, and hookah bowls are typically sized and packed for tobacco-molasses mixtures rather than cannabis alone.

Three factors specific to waterpipe use tend to reduce and change the effect compared with other cannabis-use methods:

  • Indirect, uneven heating — material can burn unevenly rather than vaporizing cleanly, so a portion of the THC may be lost to combustion rather than inhaled.
  • Water filtration — passing smoke through water can reduce the concentration of THC that reaches the user, on top of what's already lost to uneven burning.
  • Slower, more variable onset — because delivery is less direct, effects can be delayed, milder, or less consistent from one session to the next compared with methods designed specifically for cannabis.

The practical result: reaching a given level of THC effect through a hookah generally requires burning through more plant material for a less predictable result than with a method built for cannabis.

Combining nicotine and THC compounds the risks already described

Nicotine and THC both raise heart rate, so using them together in one session can produce a stronger combined cardiovascular effect than either substance alone, and has been associated with increased anxiety, nausea or palpitations in some users. Because hookah sessions run long compared with a single joint or vape, a cannabis-and-tobacco session also means more total inhaled smoke — and, if charcoal is used, more carbon monoxide exposure of the kind detailed elsewhere in this guide. Weesha doesn't sidestep the nicotine, carbon monoxide and combustion-byproduct risks already covered here; it adds THC's own effects and risks on top of them.

As with any hookah use, dizziness, marked light-headedness, nausea, confusion or fainting during a session should not be assumed to simply be "the high working." These are also recognized symptoms of carbon monoxide exposure — stop, get fresh air, and seek medical help for severe or worsening symptoms, regardless of what's in the bowl.

Legal status varies

Cannabis remains illegal or restricted in many jurisdictions and is regulated differently where it's legal. Its legal status, and any rules about where and how it can be used, depend entirely on local law.

  • Possible transmission of infections when hoses or mouthpieces are shared or poorly cleaned
  • When cannabis is added, the combined cardiovascular load of nicotine and THC together, alongside greater total smoke exposure from a longer session

Long-term waterpipe research is less extensive than the evidence base for cigarettes, partly because use patterns vary and many people use more than one tobacco product. Even with that limitation, public-health reviews link repeated hookah use with:

  • Nicotine dependence
  • Cardiovascular disease
  • Chronic respiratory symptoms and reduced lung function
  • Chronic obstructive pulmonary disease
  • Oral, lung and esophageal cancers
  • Adverse pregnancy and infant outcomes
  • Periodontal and other oral-health problems

The FDA states plainly that there are no safe tobacco products. It links hookah use with many of the same adverse outcomes as cigarette smoking, including oral, lung and esophageal cancers and chronic obstructive pulmonary disease.1

Research Snapshots and Real Clinical Cases

Controlled study: nicotine raised heart rate; charcoal drove carbon monoxide exposure

Blank and colleagues studied 37 occasional waterpipe users in randomized tobacco and flavor-matched tobacco-free sessions. Tobacco increased plasma nicotine and heart rate, while both products produced similar increases in carboxyhemoglobin and exhaled carbon monoxide. The result helps separate two parts of the "buzz": nicotine came from tobacco, while much of the carbon monoxide exposure was linked to the charcoal-heated setup.3

Clinical case series: four young adults treated for carbon monoxide poisoning

A 2014 case series described four waterpipe users aged 16 to 21. Three were hospitalized after temporarily losing consciousness. Symptomatic patients had carboxyhemoglobin levels between 20.1% and 29.6%; the patient without symptoms measured 16.7%. All improved after high-concentration oxygen treatment. The authors concluded that hookah-related carbon monoxide poisoning may be more common than clinicians and users realize.9

Hospital cohort: hookah accounted for 58 referred carbon monoxide cases

In a retrospective study of patients referred for hyperbaric oxygen therapy from 2015 through 2018, 58 of 265 analyzed carbon monoxide cases — 22% — were associated with hookah. Hookah-associated patients were younger on average than patients exposed to other carbon monoxide sources. This was a specialist referral population, so the percentage shouldn't be interpreted as the rate among all hookah users; it does show that severe exposure is clinically real.10

Dependence review: social use can still become nicotine dependence

A review of behavioral and clinical evidence found that waterpipe use can deliver dependence-producing doses of nicotine, relieve abstinence symptoms and lead to cravings and difficulty quitting. The authors also noted that dependence may change the pattern from occasional social use toward more frequent or solitary use.6

So, Does Hookah Make You High?

Traditional tobacco shisha can give you a nicotine buzz, but it doesn't create a THC high. Light-headedness may come from nicotine, carbon monoxide or both. Because carbon monoxide can reduce oxygen delivery, an intense "buzz" isn't a sign of quality or potency — it may be a safety warning.

If the product contains THC — as in a "Weesha" mixture — the high comes from THC, and a hookah tends to deliver it less efficiently than methods built for cannabis. If it contains tobacco, it can deliver addictive nicotine. If charcoal is burning, it can add substantial carbon monoxide and combustion pollutants. The water doesn't erase any of those exposures.

The most useful way to answer this question is therefore not simply "yes" or "no," but: what substance is being used, how is it being heated, and what symptoms is the person experiencing?
